As far as I know, both the current and previous versions of SpaceDock didn't offer a system to report mods.

I've seen numerous times people joining our IRC channel and reporting troll mods or mods that don't have permission to use an asset made by someone.

My idea was to have a system where the user reporting the content should describe why they are reporting the mod. We should evaluate if it's better to have a text field for manual input or a list of categories that the user can select that best define the reason they are reporting the content.
We could also include a similar system for filing a DMCA request. We should not take down a mod until we've actually confirmed that the mod violates a copyright. We shouldn't do like YouTube and GitHub that block the content before checking if the claim is valid, which leaves a breach for bots to exploit it.

Bots: submit captcha + only registered users can report anything.
Categorization of the claims is a must, otherwise it'll be difficult to handle cases. But the comment field is also a standard.

Otherwise would be trivial to implement.
