Create an audit trail for modified entries

When entries are modified, a clone of their previous state* plus metadata about the change (canonical-to timestamp, user) should be saved to a list in the entry.

Will increase weight but help with:

• Auditing changes
• Undo / restore
• Understanding data changes at a higher level (how often are things updated)

(*: maybe just the fields changed?)
