[CAL-5045] Add cancelledBy input to cancellation page

When a booking is canceled we always want to set cancelledBy. Currently, it's missing when cancelledBy in the URL is not set and user isn't logged in. For these cases, we want to show a Dialog to input your email address first. The Dialog can only be closed if you input matches an email address of the booking. Without the email input you won't be able to cancel the booking
