CacheMissed can be triggered for the same request as ResponseCacheHit

If an exception is thrown by getCachedResponseFor or one of the other methods after the ResponseCacheHit event has been triggered then a CacheMissed event will also be triggered for the same request.

This currently happens twice because of #435, so one request can have ResponseCacheHit, CacheMissed, and CacheMissed triggered.

Because of #342 this can happen quite frequently.
